FLAACOs Partners with InfoMC to Increase the Quality of Care Delivered to Patients

CONSHOHOCKEN, Pa., Oct. 17, 2018 /PRNewswire-PRWeb/ -- The Florida Association of ACOs (FLAACOs), the premier professional organization for Accountable Care Organizations and value-based healthcare leaders throughout Florida, announced today a strategic partnership with InfoMC, Inc., a national healthcare technology company with over 20 years' behavioral health-specific expertise and a leading provider of cloud-based integrated healthcare management and care coordination solutions to commercial and public-sector markets.

InfoMC's patient centric, "whole-person care" and provider integrated approach offers technology solutions that enhance interaction between the patient, primary care and behavioral health care providers to improve access to care, quality, cost, and value. Through this partnership, InfoMC will work closely with FLAACOs and the Accountable Care members to help meet their goals and overcome healthcare transformation challenges; integrating physical, behavioral and social determinants of health of the patients they serve.

InfoMC's Care Coordination Solution provides a patient-centric, multi-disciplinary approach for coordinating care for patients, and offers real-time collaboration between patients, caregivers, providers and other stakeholders. The solution is based on the CMS Model of Care and enables ACOs to effectively manage the complex physical and behavioral health needs of their populations by combining patient-centered care planning with a collaborative Care Team approach. This allows ACOs to optimally manage high-risk, complex populations while improving access, engagement, patient experience, and quality of care - all helping ACOs to improve outcomes, reduce unnecessary healthcare costs, and achieve their quality performance measures.

"So many of the problems within healthcare are the result of poor care coordination. InfoMC has a robust solution for making sure patients don't fall through the cracks and get the care they need. For ACO's managing cost and quality, this is critical for their success," says Nicole Bradberry, CEO of the Florida Association of ACOs.

"We are committed to a strong partnership supporting population health management initiatives with the Florida Association of ACOs," said JJ Farook, CEO of InfoMC. "With our commitment to improve behavioral health and health behaviors through integrated collaborative care, we truly value the dedication of ACOs to improve quality, enhance patient experience and reduce unnecessary healthcare costs while maintaining patient choice."

InfoMC Board of Directors member, John Tadich, has been invited to present a FLAACOS Webinar on October 24th addressing The Impact of Integrated Care and Social Determinants of Health; weaving whole-person health throughout the ACO framework. Mr. Tadich has over 35 years of experience in managed healthcare, working in all segments of the market including commercial programs, health plans, Medicare, and Medicaid. He has been particularly active in the behavioral health aspects of healthcare systems for the past 20 years, providing management oversight for thirteen state programs that provide behavioral health services for Medicaid and indigent populations.
