FirstVitals Health and Wellness Inc. and TimiHealth Enabling Interoperability and Care Coordination via Blockchain


AUSTIN, Texas, Oct. 16, 2018 /PRNewswire/ -- Timicoin/TimiHealth (www.timicoin.io), a healthcare Blockchain-as-a-Service company and FirstVitals Health and Wellness, Inc. have entered into the development phase to use TimiHealth's blockchain to both aggregate data across multiple electronic health records (EHRs) used by providers across the State of Hawaii and fulfill care coordination requirements of the CMS Comprehensive Primary Care Plus (CPC+) program.

FirstVitals serves approximately 100 primary care physician practices that are participating in the CPC+ program representing 40,000 attributed Medicare beneficiaries. This five year CPC+ program will run through 2021 and includes Hawaii Medical Service Association (HMSA), the Blue Cross Blue Shield of Hawaii, as a payer partner. CPC+ allows physicians to participate in one of two tracks that provides monthly care management fees on average $15 and $28 per beneficiary per month (PBPM) plus monthly quality bonuses of $2.50 or $4 PBPM, respectively for tracks 1 and 2. Additionally for the most complex 10% of the population and those with dementia, physicians under track 2 will get a $100 PBPM care management fee.

TimiHealth is mapping indices to a variety of EHRs used by FirstVitals provider partners including, Elation, Cerner, Centricity and eClinical Works. Data will be readily available via the blockchain for FirstVitals' care coordinators so that they will be able to support Medicare CPC+ care plan requirements seamlessly for care continuity, real-time access, and targeted care management. Patients will also have access and control of their own personal health data and have the option to tokenize it via TimiHealth if they so choose.
