Home care leaders meeting in Charlottetown PEI


Home-based palliative care innovations in Canada showcased at two-day event

CHARLOTTETOWN, PEI, Oct. 1, 2018 /CNW/ - Home care leaders from across Canada are meeting at the Canadian Home Care Association's (CHCA) two-day Summit on October 1st and 2nd, 2018. Kathleen Casey, Deputy Speaker and MLA for Charlottetown / Lewis Point will address delegates and lay the foundation for the Summit.  She will highlight how PEI is leveraging the targeted federal funding for home and community care to help improve access to needed health services and ultimately, improve the health and well-being of Islanders.

The 2018 Charlottetown Home Care Summittm will explore many of the challenges and opportunities facing home and community care.  Attendees will discuss new approaches and strategies to accelerate the work on the pan-Canadian priorities set out in the Common Statement of Principles on Shared Health Priorities endorsed by the federal government, provinces and territories in August 2017.

A special feature of the Summit is the live-streaming presentations of twelve leading practices in operational excellence focusing on home-based palliative care.  The identification and sharing of these practices are a key part of a national CHCA project supported through a health funding contribution agreement from Health Canada.  The project builds on the "The Way Forward: An Integrated Palliative Approach to Care" by identifying innovative operational practices to address specific service gaps and improve the quality, efficiency and accessibility of home-based palliative care.

In addition to home-based palliative care, the Summit will profile leading practices in integrated models of home care and primary health care, the INSPIRED chronic disease management program, TeamSTEPPS Canada, integrated community paramedic models and new approaches to accountability and digital health.

The CHCA is committed to connecting health care leaders to exchange knowledge and to inform our work in influencing home care policy across Canada.   The two-day event in PEI will start important conversations on how to make home care better and the pan-Canadian discussions will conclude in BC on October 22 & 23 at the western Home Care Summittm.   Stakeholder can join the conversations either in-person or virtually through the Summit live-streaming.   For more information on the CHCA Home Care Summittm visit: https://homecaresummit2018.ca
